How to Submit and Follow Up on a Service Referral

Task

Submit and manage a Service Referral through MyCollege and MyConnect

Prerequisites

Access to MyCollege is required to submit a referral.
Access to MyConnect is required to communicate with the student and complete the referral task.

Instructions

 

1. Submitting a Referral

  1. Accessing the Service Referral Form

    • Log in to MyCollege.

    • Under Additional Employee Services, select Service Referrals.

  2. Submitting the Referral Form
    • Enter your full name under Submitter Information.

    • Enter the student’s school email address (...@email.cpcc.edu)

    • Select one or more Service Departments to refer the student to.

  3. Click Submit once all fields are complete.

 

2. Following Up in MyConnect

  1. Referral Notification and Task Creation

    • The selected department(s) will receive an email with referral details.

    • A task is created in MyConnect for the department(s) to manage.
  2. Viewing and Managing the Referral Task

    • Open the referral by clicking the Task Title in your task list, or click the Student’s Name to go directly to their record.

    • The referral will appear under the Task card on the student’s MyConnect profile.
      (Note: Your screen may vary depending on your role settings.)

  3. Following Up with the Student

    • The assigned department is responsible for contacting the student.

    • To reach out, create a Conversation directly in MyConnect:

      • Select the student’s school email as the recipient.

      • Add a subject line and message.

      • Leave the From Email as-is—it defaults to the system address.

      • When you start the conversation, you are automatically added as a Participant and will receive notifications when the student replies.

      • You can also manually add yourself or others under Participants.

  4. Completing the Task

    • Once contact is made or the follow-up is complete, update the task by selecting Done under Status.

    • If needed, open the task to add notes before marking it complete.

    • A referral task is considered officially complete when its status is marked as Done.

Outcome

The student is successfully referred to the appropriate service department(s), and the assigned team follows up and completes the task in MyConnect.
